Off-label testosterone dosing, access, and lab monitoring in the US

Dorr calls the situation genuinely frustrating for clinicians, patients, and the field alike. He points to 7 randomized controlled trials from the early 2000s showing roughly 80% of women with hypoactive sexual desire disorder improved on testosterone therapy, evidence strong enough to demonstrate benefit, yet never paired with an FDA-approved formulation. Without one, every prescription in the US is necessarily off-label, typically dosed at roughly a tenth of standard male testosterone products. Dorr cautions that women are not simply scaled-down men: Skin absorption varies widely between patients on transdermal formulations, and he has seen patients who began using a partner's testosterone injections develop hair loss and other androgenic adverse events from inappropriate dosing. Watching quality, consistency, and patient compliance, he says, is as important as the prescription itself.

Uloko follows with the practical mechanics of getting dosing right. She explains that guideline math places the female daily dose at about 5 mg against a typical male dose of 50 mg or more, meaning a single tube of standard product can supply a woman for roughly 10 to 14 days, a rough estimate rather than a precise measurement, which she says makes in-office demonstration of application technique, applied to the calf, essential. She also flags a practical access issue: Patients are often better served skipping insurance entirely, since GoodRx coupons can bring a year's supply under $100, well below typical insurance pricing for a product payers frequently decline to cover.

On monitoring, Uloko favors trending labs over guessing, checking both efficacy, whether symptoms are improving and levels are adequate, and safety, tracking any adverse events against lab values to build the case-level evidence the field still lacks.
